SCHOOL TAX DEBATE As you know, Tuesday, April 20th, between 2PM and 9PM our Clubhouse is a polling location for the 2010 School Election. There is a public question that you will have the right to vote on which deals with the school budget. There has been information circulated to the community so please read it. The information shows two positions on the public question of whether or not the budget for the school system should be approved. I do not want to take a position either way since this is NOT an Emailer function. However, many residents have asked, how much is this tax increase going to cost us? Just take out your tax bill and multiply the school tax portion by 8.3%. Again, I just urge each resident to VOTE one way or another. YOU AND YOUR MEDICATIONS I guess that even though I have been retired for 12 years, the Pharmacist in me still exists. I want to make sure that each and every resident is aware of what is currently going on since it might affect some of your medications. We all know about generic substitution but many of you don't know about therapeutic substitution. First, generic substitution for most drugs is being done naturally to save insurance companies and you money. Are all the generic drugs comparable? The answer is NO - their generic names are the same but the way drugs are formulated varies from company to company. Take for example Aspirin. Lots of brands but frankly I only use Bayer. Why? They maintain the best formulation. The tablets aren't crumbled and the smell of Acetic Acid (like in some generics) is never present. If you think along these lines in regard to prescription medications and generics, you can then see that generics will vary from company to company for prescription durgs just like Aspirin. The formulations for drugs are written as standards and these standards give manufacturers some leeway when it comes to potency. A drug like Digoxin, for the heart, I would ONLY ask for Lanoxin brand. It maintains the same standards batch after batch. There are thousand of other generic drugs like Digoxin and this can be discussed with your physician or pharmacist. Keep in mind that when Pharmacy's start buying their generics from multiple companies, this might also affect a patient because you might get one brand that has an 80% potency and another brand that has a 110% potency.

What I use to tell patients is to make sure your doctor knows if there are any changes in the way you are feeling. If your blood pressure is being maintained and you are then given a different generic (from a different manufacturer) and then your pressure is either higher or lower, well it can be related to changes in the generic or it can be related to other changes you might be experiencing but the doctor should be contacted. Now for the more scary thing THERAPEUTIC SUBSTITUTION. Simply, this is like Tagamet and Zantac. They are used for the same thing but they aren't generically equivalent. The generic names are different. It's funny because I started therapeutic substitution about 14 years ago in my hospital for these two drugs. Injectable Tagament and injectable Zantac were like night and day when it came to cost. By giving pharmacists the right to subsitute one drug for another my hospital saved $1 million in those days. However, we had an extensive inservicing of the medical staff and if you used one versus the other, no problems would occur on a therapeutic basis to the patient. This is probably one reason that these drugs went from prescription to over the counter. Recently I found out that a mail order company like Caremark is now doing therapeutic substitution for prescription drugs. They called my family doctor to get the okay and fortunately my family doctor called me. They even tried to make it sound that by substituting one drug for another I would be saving lots of money. The truth is, it made no difference because Greta's copay's are $0 for any drug. So my doctor told me which drug they wanted to use and I almost "flipped". Fortunately I remembered that this "therapeutically equivalent" drug had a much higher percentage of renal toxicity then the drug my wife was getting. Yet convincing doctors based upon cost savings to the patient might make doctors say yes and this may or may not be in your best interests. So here's what I told my doctor and Caremark, I DO NOT WANT ANY THERAPEUTIC SUBSTITUTION unless I agree to it. If you are feeling okay with the drugs you are taking and you are willing to spend the difference, let your doctor know that if the insurance company wants to change your drug on a therapeutic basis, you don't want it to be done. These company's take the patient out of the decision making, which I firmly believe is wrong. Yes, you aren't a pharmacist and if you are getting a less expensive drug for a very expensive drug, you should make sure, with your doctor, that this new drug will not cause any additional problems because the adverse effects profiles of both drugs might be different. Honestly, my family doctor called me since he was just slightly familiar with the drug they were recommending. As soon as he heard about the problem, he told Caremark, NO! He might have said YES thinking that I would be saving lots of money but fortunately he knows that I might know more about the drug then he did.
